NPHI Welcomes By the Bay Health to its National Membership

[WASHINGTON D.C., January 23, 2024] – The National Partnership for Healthcare and Hospice Innovation (NPHI) is proud to announce By the Bay Health as its newest member, expanding its network of over 100 not-for-profit advanced illness, hospice, and palliative care providers.

By the Bay Health, a result of the merger between Mission Hospice & Home Care, Hope Hospice, and By the Bay Health, has been a leader in compassionate home-based health care since its inception in 1975. The organization, born out of the pioneering Hospice of Marin, was the first hospice in California and the second in the U.S. Now serving all nine counties in the San Francisco Bay Area, By the Bay Health is dedicated to providing the highest quality care and ensuring health equity for all patients.

With a multidisciplinary Care Team approach, By the Bay Health offers outstanding palliative/comfort care and skilled home health care. Their commitment to the unique and diverse needs of the San Francisco Bay Area communities is reinforced through strong partnerships with local health organizations and affiliations with UCSF Health.

As a not-for-profit, community-based organization, By the Bay Health ensures that outstanding care is accessible to everyone, regardless of their ability to pay. Donor-supported programs include caregiving grants, bereavement services, pediatric hospice care, and the annual youth grief support retreat, By the Bay Camp.
