NPHI Welcomes Centrica Care Navigators

The National Partnership for Healthcare and Hospice Innovation (NPHI), the national voice for not-for-profit hospice providers, welcomes Centrica Care Navigators as its newest member. Centrica Care Navigators, formerly Hospice Care of Southwest Michigan, serves an expanding region within Michigan. Their service areas include adult day services, end-of-life care, symptom management,
